Nope. It is not possible to complete all companions in a single playthrough. At the start of Act 2, 3/6 of the companions will die when you flee from Dallis’s ambush into the Hall of Echoes. As far as I know, all companions need Reapers Coast (act 2) and Nameless Isle (act 3) access to complete their quest.
