As with any other entity, an Item that falls into the void takes damage until it dies. The ItemDespawnEvent is only called when an item despawns after 5 minutes. You might be able to listen to the EntityDamageEvent to detect items falling into the void, but I’m not sure.

If there’s a way to disable the gamerule in a specific region for example, i don’t want them to fall into the void and lose their items while building their islands.
