The current answer is outdated. The new patches have added a Gwent difficulty that is independent from the difficulty of the rest of the game.
Does changing Gwent difficulty affect trophies?
Nope. I messed with the Gwent difficulty on my Death March playthrough because that early Gwent player in Vizima was annoying me, and still got the trophy. Only the game difficulty matters.
